About the Service
Join our team in Goulburn as a Nurse Practitioner or Transitional Nurse Practitioner - Chronic Care Management, where you will have the opportunity to make a meaningful difference to people living with chronic and complex health conditions. Working within a supportive multidisciplinary environment, you will use your advanced clinical skills to provide high-quality, person-centred care, contribute to comprehensive assessment and care planning, and support patients to better manage their health and wellbeing. Whether you are an endorsed Nurse Practitioner or a Transitional Nurse Practitioner working towards endorsement, this role offers an exciting opportunity to further develop your advanced practice skills, build strong therapeutic relationships and contribute to improved health outcomes for the Goulburn community.
