Disability Support Worker – Melbourne South East

Monash Care Disability Services is seeking reliable and compassionate Disability Support Workers to join our team across Melbourne’s South Eastern suburbs.

The role involves providing person-centred support to NDIS participants in their homes and in the community, helping them maintain independence and achieve their individual goals.

About the Role

• Assistance with Daily Living (ADLs), including showering, dressing, grooming and personal hygiene
• Meal preparation and assistance with eating where required
• Medication prompting and support in accordance with care plans
• Household tasks such as cleaning, laundry and maintaining a safe home environment
• Community access, shopping, appointments, recreational and social activities
• Transporting participants to appointments and community activities
• Supporting participants to develop and maintain independent living skills
• Providing companionship and emotional support
• Following individual support plans, behaviour support strategies and participant preferences
• Completing progress notes, incident reports and other required documentation
• Communicating effectively with participants, families and the broader support team

The role may involve a variety of shifts across Melbourne’s South East, with opportunities for additional hours as our participant base grows.

About You

We are looking for people who are dependable, respectful and genuinely interested in supporting people with disability. Previous disability support experience is highly regarded.

Ideally, you will have:

• Certificate III or IV in Individual Support, Disability, Community Services or similar, or relevant experience
• NDIS Worker Screening Check
• First Aid and CPR certification
• Current Australian driver's licence
• Reliable vehicle with comprehensive insurance
• Good communication and documentation skills
• Flexibility to work a range of shifts, including weekdays and weekends
