About the role
Path to Independence is a Queensland-based NDIS provider delivering person-centred support across Supported Independent Living (SIL) and community settings throughout Brisbane, Logan, Moreton Bay, the Gold Coast and Ipswich.

Position PurposeThe Residential Care Officer provides care, supervision and structured support to people with disability living in Supported Independent Living and other group residential settings.
The position assesses participants’ individual needs and abilities; contributes to the development, implementation and review of individual support programs; monitors participants’ progress and changing needs; and coordinates support with families, guardians, healthcare practitioners and other service providers.
The Residential Care Officer promotes participants’ safety, wellbeing, independence, skill development and community participation while maintaining a safe, stable and inclusive residential environment.
RelationshipsInternal: Disability Support Workers, Senior Team Leaders, HR, Compliance, Maintenance and other staff.
External: Allied health teams, families, GPs, hospitals, pharmacists and emergency services.
Safe and Inclusive OrganisationPath to Independence maintains strict safeguards, adhering to NDIS Worker Screening, police checks, and high standards of safety, dignity, and wellbeing for all supported individuals.
Qualifications and ExperienceQualifications: A Certificate III, IV or Diploma in Disability or Community Services is preferred. Behaviour support or mental health training is desirable.
Experience: Disability, residential or NDIS service delivery; understanding of restrictive practice guidelines.
Core Criteria: Values alignment, collaboration, NDIS knowledge, strong communication, record keeping, reporting, ICT literacy, conflict resolution, compliance with QLD screening requirements.
Key ResponsibilitiesService Delivery:
· Provide person-centred care and supervision in SIL and group residential settings.
· Assess participants’ abilities, needs, risks and support requirements.
· Implement and review individual support and skill-development programs.
· Support personal care, daily living, mobility, household activities and community participation.
· Implement individual health and Positive Behaviour Support plans.
· Monitor participants’ health, mood, behaviour, wellbeing and progress.
· Maintain safe, clean and inclusive residential environments.
Administration & Finance:
· Maintain accurate case notes, progress reports, incident reports, medication records and other participant documentation.
· Document and report changes in participants’ needs, circumstances, health or identified risks.
· Contribute to participant reviews and updates to individual support plans.
Quality & Risk:
· Identify, document and escalate participant and service-delivery risks.
· Comply with WHS, infection control, incident reporting and risk-management procedures.
· Follow NDIS Quality and Safeguards requirements and organisational policies.
People & Teams:
· Collaborate with colleagues to ensure consistent and continuous participant support.
· Liaise with families, guardians, nurses, allied health professionals and external service providers.
· Communicate relevant participant information through reports, handovers and team meetings.
· Contribute to a respectful, inclusive and supportive workplace culture.
Legal & Risk:
· Comply with applicable legislation, the NDIS Practice Standards and the NDIS Code of Conduct.
· Follow approved behaviour support plans and regulated restrictive practice requirements.
· Maintain participant privacy, dignity, safety and confidentiality.
· Work within the position’s responsibilities, training and delegated authority.
