The Team & Role
The Advance Care Planning and Voluntary Assisted Dying Liaison Officer role sits within Community Care and works across and supports both streams of the program. The Advance Care Planning Clinician works as part of a team responsible for providing advance care planning support across PH and in the community. The Voluntary Assisted Dying Liaison Officer works in collaboration with the Voluntary Assisted Dying Statewide Care Navigators to provide a coordinated response to a patient's voluntary assisted dying request, while also providing education and support to relevant clinical areas across the organisation. Success in this role requires emotional intelligence, including self-care and self-awareness, continuous learning, critical thinking and problem solving, clear and effective communication, collaboration and teamwork, consumer service, courageous conversations, accountability for success, openness to change and continuous improvement, and strong organisational skills.Please click here to see the position description.
