Advance Care Planning Clinician & Voluntary Assisted Dying Liaison Officer

Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.

The Team & Role

The Advance Care Planning and Voluntary Assisted Dying Liaison Officer role sits within Community Care and works across and supports both streams of the program. The Advance Care Planning Clinician works as part of a team responsible for providing advance care planning support across PH and in the community. The Voluntary Assisted Dying Liaison Officer works in collaboration with the Voluntary Assisted Dying Statewide Care Navigators to provide a coordinated response to a patient's voluntary assisted dying request, while also providing education and support to relevant clinical areas across the organisation. Success in this role requires emotional intelligence, including self-care and self-awareness, continuous learning, critical thinking and problem solving, clear and effective communication, collaboration and teamwork, consumer service, courageous conversations, accountability for success, openness to change and continuous improvement, and strong organisational skills.

Skills & Experience Required

• Allied Health Practitioner with current registration with Australian Health Practitioner Regulation Agency (APHRA) or eligible for registration with the relevant professional organisation (if Social Work)
• Sound understanding of Advance Care Planning aims and underlying principles
• Experience in palliative care, end-of-life care, Advance Care Planning, and or Voluntary Assisted Dying
• Demonstrated commitment to person centred care and patient advocacy
• Workplace experience supporting and mentoring colleagues / staff
