About the role
Job Description
We Care Disability Services is seeking a compassionate, experienced and dedicated Registered Nurse (RN) to join our growing team and provide high-quality clinical support to NDIS participants in the Baulkham Hills and surrounding areas.
This is an opportunity to work within a person-centred disability support environment where you can make a meaningful difference in the health, wellbeing, independence and quality of life of people living with disability.
The successful candidate will work collaboratively with participants, families, support workers and other healthcare professionals to ensure participants receive safe, individualised and clinically appropriate care.
Requirements
Conduct comprehensive health and clinical assessments for NDIS participants.Develop, implement and regularly review individualised nursing care plans.Provide clinical oversight and support for participants with complex health needs.Provide disability-related health supports within your scope of practice.Support medication management and health monitoring where required.Assess and monitor wounds, pressure areas, continence needs and other health concerns.Provide clinical advice and guidance to support workers and other team members.Train and supervise support workers in delegated health-related tasks where appropriate.Maintain accurate, confidential and up-to-date clinical documentation and case notes.Communicate effectively with participants, families, GPs, allied health professionals and other relevant stakeholders.Identify changes in a participant's health and escalate concerns appropriately.Contribute to positive health outcomes and promote participant independence and choice.Ensure all care is delivered in accordance with professional standards, organisational policies and relevant NDIS requirements.
Certifications
Bachelor of Nursing Current AHPRA RegistrationCurrent First Aid and CPR certification.Current National Police Check.Current NDIS Worker Screening Check.Current Working With Children Check, where required.
