WHO WE ARE
Russian Ethnic Representative Council of Victoria (RERC/ Russian Aged Care) is a charity and the leading community organisation founded in 1984. We provide a range of services with a strong focus on people with CALD background. Our current programs in aged care space include Support at Home (SaH), CHSP and brokerage services. RERC also acts as an umbrella for a range of community organisations to provide a platform for various cultural initiatives and community events.
ABOUT THE ROLE
The Senior Care Advisor provides leadership, direction, and operational oversight to a team of Care Advisors delivering services under the Support at Home program. The role ensures high quality, person centred, safe, and compliant care that aligns with the Aged Care Quality Standards and Support at Home (SaH) program guidelines. The Senior Care Advisor is responsible for ensuring that services are delivered efficiently, documentation is accurate, risks are managed, and clients experience high quality, culturally safe, consumer directed care.
In this role you will:
Lead, coach and support a team of Care Partners delivering care management services to SaH program participants
Oversee SaH care management operations, caseload management and service quality
Ensure compliance with Aged Care Quality Standards and Support at Home guidelines
Ensure effective functional interface is maintained between Care Advisors and clinical staff enabling strong clinical oversight and effective and safe clinical care.
Oversee new client referrals, business and operational risks related to program intakes
Drive continuous improvement, staff development and positive team culture
Conduct regular audits of client files, budgets, risk assessments, and service delivery records.
Act as escalation point for client issues, incidents and complaints.
Ensure all relevant key requirements are satisfied, and program targets are met (client intake, billable care management hours, service utilisation).
Maintain a small individual client portfolio, providing direct case management for clients with varying levels of complexity.
Build strong relationships with clients, families, community partners and other operational units of the organisation
ABOUT YOU
To be successful in this role you will need:
Demonstrated care management expertise in Support at Home/ Home Care Packages programs.
Qualification in Nursing, Allied Health or other related field coupled with a relevant clinical experience.
Demonstrated leadership experience in community or aged care settings.
Commitment to person centred care, consumer choice and cultural safety.
Strong understanding of the Aged Care Quality Standards and Support at Home program requirements.
Solution focused mindset with a continuous improvement orientation.
Excellent communication skills, confidence to manage staff and work across a range of stakeholders
Ability to manage competing priorities and work under pressure.
Cultural flexibility with proven ability to work with CALD clients.
Prior experience and confidence with care management systems.
Fluent Russian (highly desirable)
