Dementia Advisor - Aged Care at Home

Our Aged Care at Home Branch in Bundaberg is currently looking for their next Dementia Advisor to join the team   Your Role As a Dementia Advisor you will be supporting the most important people in our organisation - our clients.   Your tasks will include: Providing person centered dementia support to people living with dementia at home and their carers. Conduct comprehensive assessments, behavioural analyses, care planning, and enablement strategies. Delivering recommendations both verbally and through written reports. Coordinate care and facilitate referrals to appropriate local services and supports. Monitor and evaluate the effectiveness of interventions and support through planned reviews and follow-up assessments. Contribute to service planning, quality improvement, data collection, reporting, and other operational requirements. Build and maintain relationships with community organisations, health professionals, and key stakeholders through networking, and service promotion.   Your Attributes We are looking for a person who has what it takes to make a difference in our clients lives.   Someone who brings: Bachelors degree or equivalent qualification in a health related discipline  A positive and proactive approach to health, wellbeing, and client outcomes. Strong organisational and time management abilities A commitment to maintaining accurate, timely, and high-quality documentation.   Key Selection Criterion You will be someone who has: Demonstrated experience in delivering and coordinating community-based dementia services and supports. Experience is conducting comprehensive assessments and developing client centred care plans tailored to individual needs and preferences. Proven ability to work independently, balance competing demands and exercise sound professional judgement. Proven ability to build and maintain effective relationships with clients, families, carers, health professionals, and community and industry stakeholders. Additional Skills Required Proficient in Microsoft Office Suite, with the ability to quickly learn and adapt to new computer systems and software. Highly developed verbal, written, and interpersonal communication skills. Sound understanding of continuous quality improvement principles and practices. Strong commitment to complying with all relevant Workplace Health and Safety legislation and requirements.
