Bed Egress Clinical Nurse Consultant

Lead Patient Flow. Improve Patient Care in Princess Alexandra Hospital.

Join our Patient Flow team as a Clinical Nurse Consultant (Bed Egress) and play a pivotal role in improving patient journeys across Princess Alexandra Hospital. Working collaboratively with multidisciplinary teams, you'll lead innovative discharge planning, optimise patient flow, and ensure patients receive the right care at the right time while supporting safe, efficient access to healthcare services.

As the Clinical Nurse Consultant (Bed Egress), you'll provide advanced clinical leadership that supports timely patient discharge, improves hospital capacity, and enhances patient outcomes. You'll work alongside senior clinicians, Nurse Unit Managers and the Patient Flow Unit to coordinate care pathways, remove barriers to discharge, and champion best practice across the hospital.

In this role you will:

• Lead patient flow initiatives to improve access, capacity and patient outcomes.
• Coordinate complex discharge planning and alternative models of care.
• Provide expert clinical advice to multidisciplinary teams across the hospital.
• Identify and resolve barriers impacting timely patient discharge.
• Drive quality improvement, clinical redesign and evidence-based practice.
• Mentor and educate nursing staff while promoting contemporary clinical practice.
• Collaborate with internal and external stakeholders to deliver seamless transitions of care.
• Support service improvement initiatives aligned with patient flow performance targets.

You're an experienced Registered Nurse with exceptional clinical leadership skills and a passion for improving patient outcomes through innovative models of care. You thrive in fast-paced environments, enjoy solving complex problems, and build strong relationships that drive positive change across multidisciplinary teams.

You'll bring:

• Current AHPRA registration as a Registered Nurse.
• Extensive clinical experience with strong knowledge of patient flow and discharge planning.
• Outstanding communication, negotiation and stakeholder engagement skills.
• Proven ability to lead change, influence practice and drive service improvements.
• Strong critical thinking, clinical judgement and complex problem-solving abilities.
• Experience coordinating multidisciplinary teams to achieve shared patient outcomes.
• Commitment to evidence-based practice, quality improvement and patient-centred care.
• Postgraduate qualifications (or current study) will be highly regarded.

The Princess Alexandra Hospital (PAH) Patient Flow and Nursing Resource Service consists of key roles associated with bed resource, hospital and patient flow management, which includes the Bed Egress Clinical Nurse Consultant (CNC). Patient Flow Program assumes responsibility for managing and/ or supporting efficient, safe, and timely movement of patients from admission to discharge across the continuum of care.

Patient Flow provides governance and undertakes real-time and retrospective data analysis to monitor trends, drive service improvements, and inform decision-making. It supports staff education, maintains procedures, and ensures compliance with safety, budget, and legislative requirements
