About the role
As Support at Home Coordinator, you will work within the Aged Care Team to deliver high quality and focused care to Aged Care clients by implementing and maintaining support at home programs in collaboration with our service delivery team and third party providers.
You will act as the primary contact point between Interchange Australia, our clients and their care team, ensuring clear communication and coordinated service delivery. The position also contributes to compliance management, continuous improvement and regular travel within the local area to visit our clients.
Key responsibilities
Create and maintain client files and documentation, ensuring accuracy and updated information is on file at all times
Ensure each file is updated with reports, care plans and service agreements
Administer budgets and ensure services fit within the confines of client funding and/or group funding
Liaise with the organisation's stakeholders to communicate effectively by developing client relationships and building rapport.
About you
Minimum of 1 - 2 years in home care packages including CHSP, HCP, SAH or residential aged care coordination
Experience with care planning, budget management for client packages, and strong communication with clients, families and multidisciplinary teams.
Relevant certificates or qualifications in nursing, community service or aged care management.
Will undertake required qualification as stipulated by the Aged Care Quality & Safety Commission in order to fulfil the role
High level of written and oral communication skills
Ability to work both autonomously and as part of a team
Effective time management skills and ability to meet deadlines
Holds current NDIS Worker Screening check
Holds current Working with Children check
Current drivers licence
