The Role

The Outreach worker will provide an outreach service for the Black Swan Health Street Doctor clients and potential clients by facilitating communication, providing education and informal counselling, assisting with referrals to appropriate agencies and health care services, driving and or daily upkeep of the van to designated locations.

Duties and Responsibilities of the Position

Van and Set Up

§  Ensure that the mobile medical unit is collected for seasonal use and returned on time

§  Assist with van maintenance, including (but not limited to) adequate fuel, water pump filled, power connection and daily cleaning.

§  Emptying of clinical waste, rubbish and linen to appropriate facility.

§  Disposal of Clinical pathology and paperwork as required.

§  Other duties as required.

Clinic

§  Encourage potential clients to service

§  Greet clients and assist new clients with filling in appropriate forms.

§  Triage patients as necessary.

§  Complete Data collection form for each clinic session.

§  Make Duty Doctor and Nurse aware of any potential concerns with clients.

§  Chaperone client if requested.

§  Assist clients with Physical; Mental; Social needs (i.e. Filling in forms for Medicare Card)

§  Ascertain accommodation status – provide information regarding local hostels.

§  Ascertain access to other Community Services – provide information regarding other community services in locality.

General

§  Conduct all activities in compliance with relevant legislation including Equal Employment Opportunity, Occupational Health and Safety, and all policies, procedures and guidelines as determined by Black Swan Health.

§  Participate in Accreditation and quality improvement activities as required.

§  Represent Black Swan Health Street Doctor at community events as required.

§  Other duties as required.

The successful candidate will have

Essential

§  LR (Light Rigid) Licence.

§  Demonstrated experience or interest working with marginalised populations.

§  Previous experience providing an outreach service.

§  Understanding of barriers and challenges that disadvantaged people experience.

§  Excellent communication skills.

§  Strong understanding of confidentiality and ethical boundaries.

Desirable

§  Knowledge of local services and community organisations.

Special Conditions for Employment

§  National Police Clearance.

§  Working with Children Check.

§  Current driver’s license (LR (Light Rigid) Licence.

§  Current eligibility to work in Australia.
