Our office is located in Blacktown, with the majority of our participants living across Western Sydney and surrounding areas.
We place participant safety, rights, choice and control at the centre of our service delivery and work closely with participants, families, support workers and other professionals to provide dependable and responsive support. These principles align with Yahweh Care's Service Delivery Management SOP.
We are seeking an organised, proactive and compassionate Service Delivery Coordinator to join our Blacktown-based team.
This is a hands-on coordination role involving a combination of office-based service coordination and participant-facing activities across Western Sydney.
You will work closely with participants, families, support workers, rostering staff, Support Coordinators, allied health professionals and management to ensure participants receive consistent, safe and high-quality services.
The role may also involve attending participant homes, meetings, community activities, service introductions and other appointments across Western Sydney.
Yahweh Care company pool vehicles are available for approved work-related travel and participant community engagement activities, subject to company vehicle policies and availability.
As Service Delivery Coordinator, you will:
Coordinate the onboarding and commencement of new NDIS participants.
Maintain regular contact with participants, families and nominees.
Assist with collecting participant information, goals, routines, communication preferences and support requirements.
Coordinate participant service schedules and commencement arrangements.
Assist with service agreements, consent documentation, support information and participant records.
Coordinate suitable support worker matching based on skills, availability, compatibility, participant preferences and identified risks.
Work closely with rostering staff to maintain reliable shift coverage.
Monitor daily rosters and respond quickly to cancellations, uncovered shifts and service delivery issues.
Coordinate replacement support workers when required.
Ensure support workers are appropriately briefed regarding participant needs, routines, risks and documentation requirements.
Conduct participant and family check-ins following commencement and throughout service delivery.
Visit participants across Blacktown and the wider Western Sydney region when required.
Support participants with approved community engagement and service-related activities where appropriate.
Use Yahweh Care pool vehicles for authorised participant/community engagement and operational activities where required.
Respond to participant, family and support worker enquiries professionally and promptly.
Review progress notes and service documentation and follow up when information is incomplete.
Monitor participant satisfaction and identify opportunities to improve service delivery.
Coordinate service changes when participant circumstances, needs or NDIS plans change.
Liaise with Support Coordinators, Plan Managers, allied health professionals and other stakeholders.
Assist with participant plan review preparation and ensure relevant records are current.
Assist with incident and complaint documentation, follow-up and escalation.
Escalate participant safety, safeguarding and high-risk concerns to management immediately.
Assist with participant transitions, service exits and handovers.
Maintain accurate participant records, correspondence and operational documentation.
Support continuous improvement across participant services.
The operational responsibilities align with Yahweh Care's processes for worker matching, rostering, commencement, participant check-ins and documentation monitoring.
As most Yahweh Care participants are located throughout Western Sydney, local travel is an important part of this position.
The successful candidate should be comfortable travelling to areas including Blacktown and surrounding Western Sydney suburbs for:
Participant introductions and initial meetings
Participant welfare and service check-ins
Community engagement activities
Meetings with families and stakeholders
Support worker introductions
Service quality visits
Participant appointments or approved activities
Other service delivery requirements
Company pool cars are available for approved work-related travel and participant community engagement, reducing the need to use your personal vehicle for routine authorised company activities.

The successful candidate should have:
Previous experience in the NDIS, disability, community services, aged care or similar sector
Experience coordinating participants, clients or frontline workers
Strong communication and interpersonal skills
Strong administration and computer skills
Ability to maintain accurate participant and service records
Understanding of person-centred support
Ability and willingness to travel across Western Sydney
Current unrestricted Australian driver's licence
Confidence driving company pool vehicles for authorised work activities
NDIS Worker Screening Check or ability to obtain one
Working with Children Check where applicable
Current First Aid and CPR certification where required
Australian working rights
Qualification in Disability, Community Services, Social Work, Health or a related field
Previous experience working for an NDIS provider
Experience with participant onboarding
Experience with rostering or care-management systems
Knowledge of NDIS Practice Standards
Experience managing service delivery issues
Experience with incident, complaint and risk-management processes
Knowledge of the Blacktown and Western Sydney community
Experience supporting participants with community participation or engagement
Success in this position means participants receive reliable services, workers have the information they need, documentation remains accurate, risks are addressed promptly, and participants and families receive responsive communication.
Yahweh Care's SOP requires ongoing monitoring of rosters, cancellations, participant service quality, documentation, incidents, complaints and improvement actions.
