Graduate Registered Nurse - Community Care

We are seeking a Graduate Registered Nurse to join the team at Focus Care. Reporting to the Clinical Quality Manager, you'll gain valuable experience in community home care providing clinical care in disability and aged care settings while receiving ongoing training and mentoring.

What You'll Do

Conduct clinical assessments in clients' homes (initially under supervision)Support care planning and clinical monitoringWork alongside clients, families, GPs, allied health professionals and support workersAssist with client education and self-management strategiesParticipate in quality improvement and clinical review activitiesDevelop your skills caring for clients with diverse and complex needs

About You

Current AHPRA registration as a Registered NurseRecent graduate or early-career nursePassion for person-centred care in the communityExcellent communications skills (verbal and written)Current Driver's LicenceNDIS Worker Screening Check (or willing to obtain)
