Registered Nurse - Weekend Discharge Coordinator

The Team & Role

We have an exciting opportunity for a Registered Nurse to work as a Weekend Discharge Coordinator at our Peninsula University Hospital campus in Frankston. As a part of the Timely Emergency Care (TEC2) Program the Weekend Discharges Project aims to increase weekend discharges from Frankston Hospital.

In this role, you will be delivering high level coordination and liaison to patients nearing medical readiness for discharge to enable timely discharge home over weekends. It requires close collaboration with various healthcare professionals and consumers to address patient needs and barriers to discharge to ensure smooth and timely transitions of care. This is a fixed term part time role, working every second weekend at 24 hours per fortnight until 6 December 2026.

Skills & Experience Required

• Registered Nurse with current AHPRA registration

• Extensive experience in acute medical setting

• Workable patient flow experience i.e. has worked in charge shifts or PSM or equivalent

• Proven ability to work effectively within multidisciplinary teams
