As a Pharmacotherapy Coordinator, you will provide care coordination, health education and service navigation for people accessing pharmacotherapy and primary healthcare services. You'll work collaboratively with clinicians and support workers to deliver holistic, consumer-centred care

Appointments are subject to satisfactory completion of a Nationally Coordinated Criminal History Check, a Working with Children Check and a Pre Employment Declaration Form, together with evidence of the right to work in Australia and any required vaccinations.
Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
