About the role

In this role, the Disability Support Coordinator will monitor/implement NDIS-funded supports to help participants achieve wellbeing & community participation goals.

Your duties will include but not be limited to:

• managing daily operations of NDIS Support Coordination Services

• oversee participant assessments, care planning and referral responses

• developing collaborative, team-based approaches to participant-centered care

• partnering with participants to understand needs and create tailored action plans

• advocating for participant rights and decision-making control

• promoting independence through social inclusion initiatives.

What we're looking for

• Full Australian working rights essential

• Min. 2 years previous Disability Support Coordinator experience

• Qualification in Disability Services, Aged Care or similar

• Proven NDIS experience with strong operational guideline knowledge

• WWC Check + National Police Clearance essential

• First Aid Certificate & NDIS Workers Screening Check essential

• AUS driver's license + own vehicle and comprehensive car insurance

• Psychosocial Recover Training desirable.
