Role Description This is a full-time, on-site Social Worker role located in St Kilda, VIC. The Social Worker will provide direct support to individuals, families, and community groups through casework, counseling, and referrals to appropriate services. Daily responsibilities include conducting needs assessments, developing and implementing care plans, coordinating with local service providers, and maintaining accurate documentation and reports. The role involves facilitating group sessions, community outreach activities, and advocacy to ensure clients have equitable access to housing, healthcare, education, and employment support. The Social Worker will collaborate with multidisciplinary teams and participate in program development, monitoring, and evaluation to enhance service quality and community outcomes.
Qualifications
Formal qualification in Social Work or a closely related human services field, with eligibility for local professional registration or accreditation.Experience in case management, psychosocial assessment, and developing individualized support or care plans for diverse client populations.Knowledge of community development principles, social welfare systems, and referral pathways relevant to urban and culturally diverse communities.Strong interpersonal, communication, and counseling skills, including the ability to build trust, manage sensitive information, and work with clients in crisis.Demonstrated ability to work collaboratively in multidisciplinary teams, maintain accurate records, and use digital tools for case documentation and reporting.Understanding of trauma-informed and rights-based practice, with commitment to non-discriminatory, inclusive, and culturally safe service delivery.Experience in group facilitation, community outreach, and advocacy for vulnerable or marginalized individuals and families.Ability to manage workload, prioritize tasks, and adapt to changing community needs; fluency in relevant local languages is an advantage.
