About the role
Established in 1925, Toosey Aged and Community Care is a proud not-for-profit organisation serving the Northern Midlands region of Tasmania. Toosey provides residential aged care, community services, and independent living options for older Tasmanians in a supportive and person-centred environment. Toosey is committed to quality, compassion, and local care ensuring residents, clients and families experience dignity, respect and belonging at every stage of life.
Based in Longford and reporting to the Residential Care Manager, our Registered Nurses will be responsible for providing high-quality, person-centred nursing care to residents within our aged care facility. The role involves assessing residents' health needs, developing and implementing care plans, monitoring clinical outcomes, administering medications, and working collaboratively with residents, families, care staff, allied health professionals and other members of the Toosey team.
Key Responsibilities will include:
Championing person-centred, strengths-based care, supporting dignity, choice and independence of our residents, while ensuring care is delivered in accordance with the Strengthened Aged Care Standards, professional standards and Toosey policies and procedures.
Identifying, assessing and escalating clinical risks, incidents and deterioration appropriately.
Monitoring and respond appropriately to changes in residents' health and clinical condition.
Undertaking wound assessment and management, continence care, pain management and other clinical interventions as required.
Maintaining accurate, timely and confidential clinical documentation and resident records.
Communicating effectively with residents, their families and representatives regarding care, treatment and changes in health status.
Providing clinical guidance, support and supervision to care staff.
Supporting quality improvement initiatives including assisting with compliance and accreditation processes.
As the successful candidate, you will ideally:
Be a Registered Nurse with current AHPRA registration
Have a minimum 3 years’ clinical experience in aged care (desirable)
An understanding of the Aged Care Act 2024 and Strengthened Quality Standards.
Hold a National Police Certificate (or willingness to obtain)
