About the Role

As a Case Manager, you’ll:

• Conduct comprehensive assessments to understand each client’s unique needs.
• Collaborate with healthcare professionals and service providers to deliver tailored support.
• Build strong relationships with clients and key stakeholders.
• Ensure government funding is used effectively and in line with guidelines.

We’re Looking For

• Exceptional organisational skills and ability to self-manage.
• Strong communication skills – both written and verbal.
• Proficiency in using Microsoft Suite and client management systems.
• Commitment to working within a multidisciplinary team.

The Successful Applicant will have:

• Current Working with Vulnerable People/Children Check.
• Current National Police Check.
• Current Australian Driver’s Licence.
