About the role
The Continence Nurse is responsible for delivering person-centred, 1:1 Continence Assessments to NDIS and My Aged Care/Support at Home participants with complex health and/or disability needs. Working collaboratively with participants, families and other health professionals, the Registered/Clinical Nurse identifies continence-related needs, develops practical strategies and supports positive health, wellbeing and quality-of-life outcomes. Assessment findings are consolidated with the nurse’s clinical knowledge, experience and ongoing professional development to provide informed, individualised recommendations that align with each participant’s goals.
Key responsibilities
Assess individual participant needs, plan, implement and/or coordinate appropriate service delivery from a range of accepted options including other disciplines or agencies
Provide direct person-centred nursing care and/or individual case management to clients on a shift by shift basis in a defined clinical area with increasing autonomy over time
Plan and coordinate services with other disciplines or agencies in providing individual health care needs
Provide health support as detailed in the participants individual health plan which includes assessment and decisions for appropriate service delivery options aimed at improving patient/client health outcomes
Actively participate in monitoring client care plans and participate in clinical review programmes/audits to ensure quality patient outcomes
Maintain clinical documentation records in accordance with organisational policy
Participate in quality improvement activities that contribute to patient/client safety, risk minimisation and safe work activities within the practice setting
Contribute to the development and implementation of clinical standards, practice guidelines, protocols/audits and quality indicators
Provide health promotion and education to clients or groups and carers to improve the health outcomes of individuals within scope of Clinical Nurse
Participate in evaluative research activities within the practice setting
About you
Registered Nurse with AHPRA (Australian Health Practitioners Registration Agency)
Minimum of 5 years experience as a RN Level 1
Supporting all-encompassing components of community continence care
Highly developed clinical skills and experience in general nursing
Excellent interpersonal and communication skills including the ability to initiate and maintain contacts with team members, participants, and other service providers
A sensitive and empathetic approach to working with individuals living with complex health and or a disability and their families
Demonstrates knowledge of the Nursing and Midwifery Board of Australia Registration Standards
Working with Children Check (issued by DHS) and National Police Check – Working unsupervised with vulnerable persons
NDIS Worker Orientation Module obtained certificate of completion
Proficient in conflict management/conflict resolution in line with Code of Conduct
Sound IT proficiency (Outlook, MS Word, PDF editing and web-based booking software/tools)
