We are seeking experienced Support Coordinators to continue delivering consistent, person-centred support to participants in the Toowoomba / Brisbane region.
About the Role
You will support participants to understand and implement their NDIS plans, coordinate services, build capacity, resolve barriers and maintain accurate documentation. These roles require strong communication skills, sound judgement and the ability to work independently while managing a varied caseload.
What We’re Looking For
Minimum 12 months experience in Support Coordination - Essential
Proven ability to manage mixed caseloads independently - Essential
Strong understanding of the NDIS, plan implementation, compliance requirements and Budget Planning - Essential
Experience in a Human Services Setting
Ability to manage competing priorities and work autonomously
Relevant qualifications in Disability, Mental Health, Community Services, Social Work or Allied Health
Driver’s licence and reliable vehicle
NDIS Worker Screening Check and Blue Card
NDIS Orientation Module, NDIS Effective Communication Module
