Complex Care Support Worker

Care Aid Australia is seeking an experienced and compassionate Disability Support Worker to provide ongoing support to a participant in Altona Meadows. This role is best suited to someone with strong clinical support experience, confidence in manual handling, and a commitment to delivering high-quality, person-centred care.

About the Role

You will provide one-on-one support to a participant with complex physical and clinical support needs. The successful candidate will play a vital role in promoting the participant's independence, safety, health, and overall wellbeing while assisting with daily living activities and community participation.

Key Responsibilities

• Provide support with activities of daily living
• Perform safe manual handling transfers and mobility assistance in accordance with care plans
• Assist with hoist transfers and the use of mobility equipment where required
• Monitor and support the participant's health and clinical care requirements
• Administer or assist with medication in accordance with policies and procedures
• Support community access, appointments, and social engagement activities
• Assist with meal preparation, hydration monitoring, and household duties
• Accurately complete shift notes, progress reports, and incident documentation
• Follow support plans, risk assessments, and health management plans
• Work collaboratively with family members, allied health professionals, and the wider support team
• Maintain a safe, respectful, and person-centred approach at all times

Essential Requirements

• Previous experience supporting participants with physical disabilities and clinical care needs
• Demonstrated experience in manual handling, transfers, and the use of disability support equipment
• Experience with medication assistance, and health monitoring
• Strong understanding of person-centred support practices
• Excellent communication and report-writing skills
• Ability to work independently and respond effectively to changing support needs
• Current NDIS Worker Screening Check
• Current Working with Children Check (or willingness to obtain)
• Current First Aid and CPR Certification
• Valid Australian Driver's Licence and reliable vehicle
• Australian working rights

Desirable Skills & Experience

• Qualification in Disability, Individual Support, Nursing, Aged Care, or a related field
• Experience supporting participants with physical disabilities
• Experience in manual handling, transfers, and the use of disability support equipment
• Experience working within the NDIS sector
