About the Opportunity:

Are you passionate about making a meaningful impact while working in a dynamic and supportive team? Puntukurnu Aboriginal Medical Services (PAMS) is seeking Social Emotional Wellbeing Worker- Culture Care Connect(Male) to join our organisation.

Culture Care Connect is a collaborative partnership between Mawarnkarra Health Service, Puntukurnu Aboriginal Medical Service, and Wirraka Maya Health Service Aboriginal Corporation, delivering culturally responsive aftercare services across the Pilbara. The program supports Aboriginal and Torres Strait Islander people following a suicide attempt or suicidal crisis by providing a safe, supportive environment, culturally appropriate care coordination, and connections to relevant health, social, and community services.

This role focuses on delivering coordinated immediate, short-term, and medium-term interventions tailored to individual needs, ensuring ongoing support, continuity of care, and improved wellbeing outcomes for clients and their families.

Key Responsibilities:

·Provide culturally safe aftercare support to Aboriginal and Torres Strait Islander people following a suicide attempt or suicidal crisis.

·Deliver timely outreach, follow-up support, risk assessment, safety planning, and care coordination.

·Assist clients to access health, social, and community support services and attend appointments.

·Build and maintain strong partnerships with health, mental health, education, emergency, and community service providers.

·Develop and monitor individual care plans to support recovery and wellbeing.

·Participate in community networks, continuous quality improvement activities, and reporting requirements.

·Work collaboratively with clients, communities, and stakeholders to strengthen suicide prevention and early intervention outcomes.

·Uphold organisational policies, confidentiality, workplace health and safety, and professional standards.

About You:

To be considered for this position, you must have:

·Aboriginality is an essential requirement under Section 50(d) of the Equal Opportunity Act 1984 (WA).

·Experience engaging and communicating with clients, communities, stakeholders, and service providers.

·Understanding of the social and emotional wellbeing issues affecting Aboriginal children, individuals, and families.

·Strong written, verbal, and interpersonal communication skills.

·Ability to work independently, manage competing priorities, and contribute effectively in a team environment.

·Proficiency in Microsoft Office and electronic client/patient management systems.

·Knowledge of relevant legislation, policy frameworks, and Primary Health Care principles.

·Commitment to culturally safe practice, professional development, and positive outcomes for Aboriginal and Torres Strait Islander peoples.

The position involves working with a multicultural organisation where the majority of staff, clients and stakeholders identify as Aboriginal or Torres Strait Islander. Candidates must be able to demonstrate an understanding of the issues affecting Aboriginal and Torres Strait Islander people and their ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people.

About the Organisation

With an administrative hub based in Newman, in the Pilbara region of Western Australia, The Puntukurnu Aboriginal Medical Service (PAMS) provides holistic primary health care to the individuals and families within the remote communities of Jigalong, Parnngurr, Punmu and Kunawarritji.

PAMS aims to improve the standard of health of the Martu people and all Aboriginal people living in the East Pilbara region of Western Australia through the provision of community based, comprehensive primary health care services. We exist to promote the health and well-being of individual community members and to relieve sickness and the suffering of the indigenous community as a whole. We are passionate about providing the highest standard of patient care whilst incorporating a holistic approach toward diagnosis and management of illness.
