About the role
We're looking for a friendly and passionate Registered Nurse to join our team on a full-time basis at Myhealth Auburn Clinic. This is a fantastic opportunity for a nurse who enjoys building long-term patient relationships and improving health outcomes through preventative care and chronic disease management.

Our position offers a Monday to Friday schedule during office hours, allowing you to achieve a fulfilling work-life balance.

A day in the life:

• Work closely with GPs to deliver high-quality Chronic Disease Management services
• Coordinate and manage GP Management Plans (Care Plans), Reviews, Team Care Arrangements (TCAs) and Health Assessments
• Drive Chronic Care Management performance across the clinic
• Mentor and support the nursing team in Chronic Disease Management processes
• Build nurturing relationships with patients to encourage engagement in ongoing healthcare
• Maintain accurate clinical documentation and patient records
• Other related duties as required

What we're looking for:

• Current AHPRA Registration as a Registered Nurse
• Previous General Practice nursing experience (preferred)
• Demonstrated experience in Chronic Disease Management, including Care Plans, Reviews, TCAs and Health Assessments
• Excellent communication and strong customer service skills
• Ability to work autonomously when required
• Strong multitasking abilities and effective prioritisation of workflow
• Best Practice software experience (desirable)
