Registered Nurse – Community / Home‑Based Care

We are seeking experienced and patient‑focused Registered Nurses to deliver in‑home education and sub‑cutaneous therapy support for patients prescribed specialist medications.

This is a flexible, community‑based opportunity suited to nurses who enjoy autonomy, patient education, and working one‑on‑one in the home environment.

About the Role

In this role, you will support patients and/or their carers through:

• Education and training on self‑administered sub‑cutaneous injections

• In‑home sub‑cutaneous infusions, including pre‑ and post‑observations

• Documentation, reporting, and adverse event monitoring in line with program requirements

Visit Expectations

• Patient contact to be made within 48 hours of referral to arrange a mutually convenient visit

• Visit duration:

  • Injection training: approx. 1.5 hours

  • Infusion visits: approx. 2.5 hours

• Completion of visit documentation and time recording within 24 hours via a secure online portal

• Prompt reporting of any adverse events, incidents, or spills in accordance with protocols

Training Provided

• Comprehensive online training program covering:

  • Program overview

  • Disease and medication education

  • Medication administration

  • Pharmacovigilance and adverse event reporting

  • Standard operating procedures

Essential Requirements

• Current Registered Nurse registration (AHPRA)

• Current CPR certification

• Demonstrated experience with Sub‑cutaneous injections AND IV cannulation

• High attention to detail and strong clinical documentation skills

• Excellent communication and interpersonal skills

• High level of computer literacy and confidence using online portals

• Flexible availability (days and times)

• Access to a safe, roadworthy vehicle

• Current unrestricted Australian driver’s licence

Desirable Experience

• Community or home‑based nursing experience
