Wesley Mission is seeking a passionate and experienced Case Manager to join our Intensive Therapeutic Care – Significant Disability (ITC‑SD) team. About the Program

The ITC‑SD program provides therapeutic residential care to children and young people diagnosed with a significant disability in statutory Out‑of‑Home Care. Our team delivers trauma‑informed, person‑centred support with a strong focus on permanency, recovery and life‑skill development.

About the role

As Case Manager, you will deliver high‑quality case work services to children and young people in Intensive Therapeutic Care. You will lead permanency planning, develop and review case plans, and work collaboratively with therapeutic specialists, house managers and support workers.

This role is fast‑paced, rewarding and deeply impactful — ideal for someone who thrives in a multidisciplinary environment and is committed to child‑centred practice.

Key responsibilities include:

• Developing, implementing and reviewing therapeutic case plans
• Facilitating case reviews and completing all required documentation
• Supporting identity development, life story work and life skills
• Building strong partnerships with Aboriginal, CALD, specialist and mainstream services
• Ensuring compliance with OCG Child Safe Standards and DCJ OOHC Case Management Policy
• Maintaining accurate records and contributing to continuous improvement

Essential criteria

• Bachelor degree in Social Work, Psychology or Social Welfare
• Minimum 2 years’ casework or Out‑of‑Home Care experience
• Experience supporting young people with complex care needs
• Strong understanding of OOHC policy, standards and quality frameworks
• Excellent communication, assessment and planning skills
• Ability to work independently and within a multidisciplinary team
• Current unrestricted NSW driver licence
• Working With Children Check and satisfactory criminal history check

• Salary in accordance with SCHADS Award 2010, Level 5
