About the role
The ideal candidate is an experienced and collaborative clinician with strong leadership skills and a commitment to improving outcomes for people with disability. The Team Leader will provide direction, build team capability and help shape the ongoing development of the Disability Resource Team. They have advanced knowledge of the disability sector, including the NDIS, disability legislation, service pathways and the interface between health and community systems. They confidently lead and support staff through clinical consultation, supervision, reflective practice and professional development, while fostering a positive, accountable and high-performing team culture. The right person has experience supporting people with disability with complex health and behavioural support needs and can apply reasonable adjustment principles to promote safe, person-centred care. They build strong relationships with clinicians, managers, government agencies and external stakeholders to coordinate care, identify service gaps and improve transitions between hospital and community settings. They are skilled in leading service improvements, managing risks, and using evidence, data and collaboration to strengthen disability inclusion, quality and access to care across SWSLHD. Adaptable and committed to equitable care, they uphold CORE values and support the delivery of safe, coordinated and culturally responsive services.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
Relevant qualification/ registration in one of the disciplines, as noted in the essential criteria (please refer to the position description)
Demonstrate your advanced knowledge and understanding of the health and disability sectors as a health clinician, including the National Disability Insurance Scheme (NDIS), relevant disability legislation, service pathways and the interface between health, community and social care systems. Please provide examples of how you have led or provided clinical oversight, applied reasonable adjustment principles, supported staff capability and reflective practice, managed risks and achieved person-centred outcomes in supporting people with disability, particularly those exhibiting behaviours of concern.
Provide an example of a disability-related project, service improvement initiative, pathway development activity or change process that you have led within a health setting. Describe how you identified a service gap, risk or system challenge; engaged and collaborated with key stakeholders to develop solutions; and implemented, monitored and evaluated the initiative. Outline how you measured outcomes, used data to inform decision-making, and improved disability inclusion, safety, quality, access to care and/or service performance.
Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
