We have 2 x amazing opportunities for Care Facilitator servicing clients in the following areas; Dubbo & Coonamble. You will assist clients and their families to engage in services, enabling them to remain living in their home. You will empower our clients through individualised care planning and goal-setting focused on our well-living approach.

You will need to be comfortable working on your own in the community, have excellent verbal communication skills and have an empathetic, patient and caring nature.
