Support Coordinator

This 6 month contract with possibility for extension services the NSW/Wollongong area. As such, applicants must be based within the local area and comfortable working within the community.

Are you an experienced Support Coordinator or NDIS Planner looking to take the next step in your career with a growing and reputable organisation?

As a Support Coordinator, you'll play a vital role in empowering people with disability to navigate the NDIS and maximise the benefits of their plans. Working closely with participants, families, carers, and service providers, you'll coordinate tailored supports, build strong networks, and help individuals achieve meaningful outcomes that enhance their independence and quality of life.

What You’ll Do

•    Support clients and families in accessing and coordinating services.

•    Build relationships with internal and external stakeholders.

•    Implement supports outlined in NDIS plans.

•    Help clients develop skills to coordinate support and engage in the community.

•    Empower clients to understand and implement their plans.

•    Be responsive to client needs, especially in crises.

•    Conduct regular progress reviews and report to the NDIS.

What You Bring

•    6-12 months experience in Support Coordination

·       Relevant industry experience or qualification in disability services, social work or similar

•    Demonstrated understanding or knowledge of the NDIS

•    Familiarity with psychosocial disabilities (advantage)

•    Strong communication skills

•    Demonstrable problem-solving skills

Essential Requirements

•    Valid Driver’s Licence

•    Paid NDIS Worker Screening Check – or be willing to obtain

•    Paid NSW Working with Children Check - or be willing to obtain

•    NDIS Orientation Module Certificate: “Quality, Safety & You”

•    Australian Working Rights (with no restrictions)
