Service Coordinator – Service Access Team

The Opportunity
We are looking for a Service Coordinator to join our supportive and professional multidisciplinary Service Access team in a flexible part time capacity 0.4FTE. 

Responsibilities include but are not limited to;

• Processing & triage incoming referrals for Counselling, Family Violence, Children’s team, Adult Allied health.  Mainly phone based
• The Service coordinator will complete initial assessment for general and family violence counselling, including screening for risk of suicide, self-harm, and harm to and from others. The Service Coordinator will possess an empathic and professional engagement style. healthAbility is not a crisis service, the Service Coordinator will respond to risk where acute and out of scope redirecting referrals to emergency and crisis services. Demonstrated experience and training in risk screening and responding to risk with consumers is mandatory.
• Engage with provided training (E.g., ASIST, MARAM) for screening of risk factors, supporting clients short-term via referral, education, and planning for safety.
• Ensure the service coordination system is responsive to the health and wellbeing needs of people.
• Provide high quality service coordination to clients.
• Engage with provided training (E.g., ASIST, MARAM) for screening of risk factors, supporting clients short-term via referral, education, and planning for safety.
• Prioritisation of priority or complex client referrals and enquiries based on sound clinical triage and assessment.
• Facilitate streamlined and timely access for clients to the services they require.
• Support internal service providers to make appropriate referrals.
• Support external agencies and referrers, including Carers, Council, MAC, GP’s, Medical Specialty, Acute Services, Health Services to refer to CH.
• Use the required client management systems e.g., Vantage and/or My Aged Care.

Skills & Experience

• Experience triaging counselling, family violence and other health related referrals in a community health setting
• Commitment to the provision of high-quality community health services with an excellent understanding of social and health issues, for best client outcomes.
• High-level understanding of the Social Determinants of Health.
• High level communication, assessment and interpersonal skills, ability to establish relationships with clients and colleagues.
• Necessary skills and attributes to manage change positively.
• Demonstrated ability to be adaptable and flexible.
• At least two years’ experience ideally in health, mental health, or community setting, involving risk assessment and problem-solving skills.
• Willingness to respond to the needs of clients from culturally and/or linguistically diverse backgrounds.
• Ability to manage a case load, to prioritise and meet deadlines, within a team or independently.
• Capacity to take initiative and engage in the planning, implementation, and evaluation of services and/or programs.
