About the role:
Western Health is seeking an experienced and compassionate clinician to join our Emergency Department Care Coordination (EDCC) Team on a 12-month fixed-term, full-time basis. This is an exciting opportunity to work within a dynamic, multidisciplinary service that supports some of the most vulnerable members of our community by delivering innovative, person-centred care within the Emergency Department.
As an Emergency Department Care Coordinator, you will play a key role in improving patient outcomes by reducing avoidable hospital admissions, facilitating safe and timely discharges, and coordinating care for patients with complex medical, psychological and social needs.
Key responsibilities:
Working within a transdisciplinary team, you will provide comprehensive care coordination for patients presenting with complex needs, including those experiencing:
You will collaborate closely with emergency clinicians, inpatient teams, community providers and external agencies to remove barriers to discharge, improve continuity of care and support patients to access appropriate services within the community.
