About Us
Since its establishment in 1982, Alzheimer’s WA has served as a prominent advocate and source of support for individuals living with dementia, as well as their families and caregivers. As we progress and enhance our services, we are moving forward in a constructive and promising direction, championing person-centred care, advocacy, education, and comprehensive support through our Commonwealth Home Support Programme (CHSP) and Support at Home (SAH) packages, in addition to providing day and overnight respite services across Perth, Mandurah, and Albany. Our commitment is to accompany those affected by dementia and effect meaningful change each day.

What We Offer
About the Role
We are seeking an experienced Support At Home (SAH) Coordinator with the enthusiasm, care, and compassion to support people living with dementia based in Shenton Park.
This position coordinates the individualized supports and services for our clients from onboarding, through to review, to enable the client’s needs and goals to be met, throughout their dementia journey.
