The Role
Our Health Navigation team launched in August 2023 when our Hobart Wellness Hub opened its doors and expanded its services in 2024 at our Launceston Wellness Hub. Since that time, St Lukes has been helping Tasmanians at a grassroots level by providing support and direction in navigating the Tasmanian health ecosystem, empowering them to manage their health care journey with confidence.
Based in Hobart, this Health Navigator role is a unique opportunity to make a meaningful difference in the lives of Tasmanians. Working through personalised, one-on-one consultations, you will listen with care, help participants identify and overcome barriers, and use health literacy principles to empower them to better understand their options, make sense of complex health information, and confidently navigate the health and wellbeing services available to them. This is practical, person-centred work that builds confidence, connection and capability. You will also support St Lukes’ broader community impact by helping develop business-to-business referral pathways, delivering group facilitation, and representing Health Navigation through off-site pop-ups.
To be successful in this role, you will have:
A clear understanding of the health ecosystem through lived experience or relevant experience in a health-related field (ideally physical health-related).
Experience in group facilitation and delivering presentations.
Exceptional communication skills and ability to quickly build rapport with a diverse range of stakeholders to facilitate provision of appropriate multidisciplinary care and support for people in their health journey.
Outcomes and value focused attitude in advancing the collective interest of our members, driving a culture that puts the members at the centre of all activity.
An ability to think logically, solve problems and understand process in multidisciplinary healthcare setting.
Proven and effective written and verbal communication, along with highly developed interpersonal skills
Proficient in use of relevant IT and software applications
