About the role
As a Supportive Care Navigator, you’ll be part of the Leukaemia Foundation’s Triage & Navigation team, handling inbound referrals and phone calls from individuals and healthcare professionals. By conducting comprehensive intake assessments, you’ll offer initial support and connect clients to relevant information and ongoing supportive care.
Your mission is to appropriately triage, coordinate, and facilitate client access to the Leukaemia Foundation’s full suite of supportive care services, including digital, 1:1, and practical supports such as financial assistance.
You’ll work collaboratively with healthcare professionals, both internal and external, and maintain accurate, up-to-date healthcare records that comply with organisational policies, regulatory requirements, and health service standards.
