We are seeking a motivated and compassionate Community Physiotherapist to join our growing team. This is an excellent opportunity for a physiotherapist who enjoys working independently in the community while remaining part of a supportive and experienced team.
You will provide in-home physiotherapy services to clients receiving support through the Support at Home Program and to NDIS participants, helping clients maintain or improve their mobility, independence, safety and overall quality of life.
You will be based at our Mount Waverley office and will travel to clients throughout metropolitan Melbourne and the Bass Coast region.
This is a full-time position, with flexible working arrangements available.
Salary
$80,000 – $90,000 per annum + superannuation, depending on qualifications and experience.
Key Responsibilities
Your responsibilities will include:
Conduct comprehensive physiotherapy assessments, including general assessment (e.g. strength, balance, gait, etc), post-fall review, post-hospital admission review, pain review and Assistive Technology assessment.
Develop and implement individualised physiotherapy care plans aligned with each client's goals, needs and available funding.
Provide evidence-based physiotherapy interventions, including:
Strength and balance training
Falls prevention programs
Mobility and gait retraining
Pain management
Chronic condition management
Neurological rehabilitation
Manual handling assessment, strategies and training
Manual therapy and relevant hands-on treatments
Assistive technology and mobility equipment recommendations
Home equipment prescription and recommendations
Monitor client progress and outcomes and adjust treatment plans as required.
Maintain accurate, timely and professional clinical documentation.
Work collaboratively with Care Partners, Registered Nurses, support workers and other allied health professionals.
Provide education and advice to clients, families, carers and support workers where appropriate.
Participate in case conferences, care planning and service reviews.
Support at Home Clients
Contribute to client care planning, reviews and goal setting.
Deliver physiotherapy services in accordance with the Aged Care Quality Standards and Support at Home requirements.
Support clients to maintain independence, mobility, safety and participation within their home and community.
NDIS Participants
Deliver physiotherapy interventions aligned with participant goals and NDIS funding.
Prepare high-quality clinical reports where required.
Contribute to Functional Capacity Assessments where appropriate.
Prepare recommendations and supporting documentation for assistive technology and equipment.
Ensure services are delivered in accordance with relevant NDIS Practice Standards.
Skills and Experience
To be successful in this role, you will have:
A Bachelor or master’s degree in Physiotherapy.
Current registration with AHPRA.
A minimum of 1 year of physiotherapy experience.
Strong clinical assessment, treatment planning, documentation and report writing skills
Ability to work autonomously and effectively manage a community-based caseload.
Excellent communication, interpersonal and organisational skills.
A client-centered approach with a strong commitment to promoting independence and quality of life.
Highly Desirable
At least 1 year of experience working in the community sector, aged care sector and/or private practice sector.
Experience working with older people, people living with disability or clients with complex care needs.
Knowledge of the Support at Home Program, aged care service delivery and Aged Care Quality Standards.
Understanding of the NDIS Practice Standards, participant goals and funding arrangements.
Experience in falls prevention.
Experience in chronic disease management.
Experience in neurological rehabilitation.
Manual handling assessment or training credentials.
Experience prescribing mobility aids, assistive technology and home equipment.
Other Requirements
The successful applicant will be required to have:
Current Victorian Driver Licence.
Reliable vehicle with appropriate insurance.
Current Police Check and/or NDIS Worker Screening Check.
Current First Aid Certificate.
Current CPR Certificate.
