Join South Western Sydney Local Health District's Acute and Extended Care Community Nursing teams and make a meaningful difference by providing safe, high-quality, person-centred care to patients in their own homes and community clinics.As a Registered Nurse, you will care for patients with a range of complex acute, chronic and palliative care needs, working collaboratively with patients, families, carers and the multidisciplinary team to achieve positive health outcomes.This role provides an opportunity to support hospital avoidance, facilitate timely discharge and enable patients to receive quality care closer to home, promoting independence and continuity of care.You will have the opportunity to broaden your clinical knowledge and develop your community nursing skills across a diverse range of clinical presentations, with support from experienced nursing and multidisciplinary teams.

We are looking for a compassionate and motivated Registered Nurse who is committed to delivering safe, high-quality and person-centred care and is passionate about making a difference in the community.The successful applicant will demonstrate:Strong clinical assessment, decision-making and critical thinking skills.Ability to work independently and confidently across home-visiting and community clinic environments.Excellent communication and interpersonal skills, with the ability to build positive relationships with patients, families, carers and colleagues.Ability to work effectively as part of a multidisciplinary team.Flexibility, adaptability and resilience when responding to varied and changing patient needs.Sound clinical judgement and a strong commitment to patient safety and positive health outcomes.Ability to provide evidence-based nursing care and manage patients with complex health needs.Commitment to professional development, accountability and continuous improvement.Experience or demonstrated skills in areas such as Hospital in the Home, wound care, PIVC/CVAD, surgical drains, catheter care and palliative care will be highly regarded. An interest and willingness to further develop skills in community nursing is also valued.

Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA).What is your understanding of Community Nursing, and how would you apply this understanding to provide safe, person-centred care to patients with acute, chronic and/or palliative care needs in their home or community setting?Provide an example of how you have used clinical assessment, critical thinking and evidence-based practice to identify and respond to a patient's changing health needs. What was the outcome?Describe how you have effectively communicated and worked with patients, carers and the multidisciplinary team to ensure care needs were identified and addressed. How did you manage any challenges or conflict that arose?Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
