About the role
Aged Care Services Intake Officer
Help people access the support they need to live well at home
Western District Health Service is seeking a compassionate and organised Aged Care Services Intake Officer to join our Aged Care Services team. This role is based in Hamilton and supports clients across the Support at Home Program, Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACC-PYP).
As the first point of contact for many clients entering our services, you will play a vital role in coordinating referrals, conducting assessments, developing care plans and connecting people with the supports they need to maintain their independence, wellbeing and quality of life.

What You'll Be Doing
Client Assessment & Care Planning
Conduct comprehensive client assessments to identify individual goals, needs, preferences and risks, developing person-centred care plans that support independence and wellbeing.
Intake & Service Coordination
Manage referrals across the Support at Home Program, CHSP and HACC-PYP, ensuring services commence within required timeframes and clients receive timely access to appropriate supports.
Client & Family Engagement
Build positive relationships with clients, families, carers and representatives, providing information, guidance and support throughout the intake and assessment process.
Care Plan Reviews
Monitor changing client needs through regular reviews and implement adjustments to services and care plans to ensure ongoing quality outcomes.
Documentation & Systems Management
Maintain accurate client records, care plans, budgets and service documentation using systems including My Aged Care, eSAH, PRODA and other client management platforms.
Collaboration & Quality Improvement
Work closely with internal teams and external service providers to coordinate service delivery while contributing to continuous improvement initiatives and best-practice service outcomes.
