Key Responsibilities 

• Planning, managing, coordinating, and reviewing client goals.  

• Actively communicate with team members and clients to develop care plans.

• Formalise agreements with the client and client’s family about the care services to be provided and costs involved.  

• Accurately document all interactions held with client, family or other stakeholders about the client and share relevant information with appropriate team members including feedback about caregiver performance.  

• Meet with clients and their families to review care plans, discuss goals, and ensure services continue to meet their needs.

• Managing a caseload of up to 50 clients with regular face to face client visits.

About you 

• Confident using Microsoft Office and database systems, with solid administration experience.

• Well organised, resilient and able to manage competing priorities.

• Ready to take on and manage a caseload in a busy, rewarding environment.

• A positive, can do attitude with a willingness to learn and grow.

• Qualifications in Community Services, Health, Aged Care or a related field are desirable but not essential.
