Intensive Case Manager (ICMI)

Help create lasting change for people experiencing homelessness.

As an Intensive Case Manager at Meli, you will work alongside individuals with high and complex needs to support them in achieving stability and rebuilding their lives. This role is deeply relational and hands-on, combining assertive outreach, advocacy, and tailored case management to meet people where they are, both physically and emotionally. You’ll play a critical role in helping clients secure and sustain housing, navigate complex service systems, and develop the skills and confidence needed for long-term independence. Working as part of a collaborative service network, your efforts will directly contribute to stronger outcomes for individuals and a more responsive, connected community.

As an Intensive Case Manager, you will:

• Provide intensive, client-centred case management to adults experiencing homelessness, building strong, trusting relationships to support long-term stability
• Deliver assertive outreach and flexible support in a variety of settings, engaging clients in their everyday environments and responding to crisis situations as they arise
• Advocate for and coordinate access to housing, health, and specialist services, working collaboratively across sectors to achieve positive outcomes
• Develop and implement tailored case plans using trauma-informed and strengths-based approaches, supporting clients to build independence, resilience, and sustainable pathways out of homelessness

Position Details

• Part time, fixed term position until 02 July 2027
• 0.8 FTE working 30.4 hours per week
• $105,834 p/a (pro-rata) + Super + Salary Packaging

What We’re Looking For
We value individuals who bring dedication, empathy, and expertise to their roles. To succeed in this position, you will have:

• Tertiary qualification (Diploma or higher) in Social Work, Community Services, Psychology, or a related discipline.
• Demonstrated experience case managing young people and adults experiencing homelessness, family violence, and other complex needs, with the ability to adapt support approaches to individual circumstances.
• Strong knowledge of the housing and homelessness sector, including renter rights under the Residential Tenancies Act 1997, and the ability to advocate effectively for positive housing outcomes.
• Proven ability to work respectfully and inclusively with diverse communities, including CALD, LGBTIQ+, and First Nations peoples.
• Well-developed assessment, communication, and stakeholder engagement skills, with the ability to rapidly identify support needs and collaborate with internal and external services to achieve positive client outcomes.
