Care Navigator

This is a vacancy for a Fixed Term Part Time appointment at 68.40 hours per fortnight, with the possibility of extension and/or permanency.

We are currently seeking a Care Navigator to join our team.The Care Navigator works with patients and support networks in collaboration with a person’s existing services and local providers to ensure coordinated service delivery, provision of appropriate information, education and support for Voluntary Assisted Dying (VAD), providing care navigation and case management. Acts as a role model and consultant to other health service providers and health care workers (in both the Perth metropolitan and regional areas), providing knowledge, support and clinical expertise in the area of VAD. As a statewide service, requirement for regional travel is a routine part of this role.
