Key Responsibilities

• Deliver high-quality Level 2 and Level 3 Support Coordination

• Support participants to understand and implement their NDIS plans

• Build and maintain strong relationships with participants, families, and carers

• Liaise with service providers and government agencies

• Maintain accurate and compliant case notes and documentation

• Work autonomously while being part of a supportive team

Essential Criteria

• Proven experience as a Level 2 and/or Level 3 Support Coordinator

• Minimum of 1 year experience in NDIS Support Coordination

• Level 2 Support Coordination experience is required as minimum qualification

• Strong knowledge of the NDIS and service system

• Current NDIS Worker Screening Check

• Working With Children Check (WWCC)

• Valid Australian Driver's Licence

• Excellent communication and time management skills

• Ability to manage a full caseload independently
