Support Worker – Transitional Care (Residential Setting)
Tea Cup Cottage Pty Ltd | Birkdale, Brisbane QLD | Casual | $40–$43 per hour

From residential and respite care to community programs and in-home support, everything we do helps people live meaningful, connected lives.
We're looking for compassionate, capable Support Workers to join our Transitional Care team, supporting participants as they move from acute hospital care into a safe, supportive home environment. If you lead with empathy, thrive in a team, and want your work to genuinely matter — we want to hear from you.
What you'll be doingWorking alongside our Registered Nurses and Clinical team, you'll help participants with daily living, lifestyle activities and evidence-based care, including:
• Personal care, mobility, hygiene, toileting and feeding support
• Medication and health-related support activities
• Social engagement, lifestyle goals and community access
• Supporting participants through hospital admissions and discharges
• Contributing to care planning and participant goal outcomes
• Keeping the home safe and welcoming through light domestic duties
• Accurate documentation and shift handovers
• Collaborating within a multidisciplinary team, and growing through ongoing training and team meetings
What we're looking for• A compassionate, reliable and professional approach
• Confidence working in a residential care environment
• Strong, collaborative communication
• A genuine passion for person-centred support
• Adaptability in a fast-paced, evolving care setting
• A commitment to dignity, respect and quality outcomes
What you'll need• Certificate III or IV in Individual Support, Disability, Aged Care or similar (preferred)
• Experience in disability, aged care, transitional care or residential support
• Current First Aid & CPR
• Current NDIS Worker Screening Clearance
• Blue Card (or willingness to obtain one)
• Valid Australian driver's licence
• Solid documentation and computer skills
• Availability across a rotating roster, including weekends — morning and afternoon shifts only; sleepovers are only needed if we onboard complex clients
