Senior Clinician - Aged Care Assessment

About the role:

The Western Aged Care Assessment (ACA) service plays a vital role within Western Health at Home, supporting older people with care needs to access appropriate Commonwealth-funded aged care services. Assessors undertake comprehensive clinical and non-clinical assessments, develop person-centred support plans, and coordinate referrals, reviews and assessment activities to ensure individuals receive the care that best meets their needs and preferences.

The ACA Senior Clinician provides clinical leadership, expert advice, and professional support to the Aged Care Assessment (ACA) team, Western Health at Home, and key health and community stakeholders across Melbourne's Western Metropolitan region. The role includes providing clinical supervision to multidisciplinary Aged Care Assessors, leading quality improvement initiatives, supporting service development, and ensuring the delivery of high-quality, person-centred aged care assessment services. We are seeking a motivated and collaborative clinician with strong leadership, communication, and clinical skills, who demonstrates integrity, sound judgement, and a commitment to excellence in care.

Key responsibilities:

• Provide clinical leadership, supervision, and mentoring to a multidisciplinary team of Aged Care Assessors, fostering excellence in person-centred practice.
• Undertake and support complex aged care assessments across community, hospital, and other care settings, applying advanced clinical reasoning and evidence-based decision-making.
• Coordinate day-to-day service delivery, including assessment allocation, case conferencing, reflective practice, and support for complex client care.
• Lead quality improvement, workforce development, education, and service enhancement initiatives in collaboration with the Service Manager and key stakeholders.
• Promote best practice by maintaining expert knowledge of My Aged Care, aged care legislation, assessment frameworks, and clinical governance, while building strong partnerships across health and community services.

About you:

• Relevant qualification as a Registered Nurse (Division 1), Occupational Therapist, Physiotherapist, Dietitian, Speech Pathology or Social Worker, with current professional registration (or eligibility for AASW membership for Social Workers) and a current Victorian Driver Licence.
• Minimum seven years' clinical experience, including at least two years as an Aged Care Assessment (ACA) Clinical Assessor, with demonstrated expertise across ACA clinical functions and My Aged Care.
• Strong clinical leadership, independent decision-making, and advanced assessment skills, with a commitment to evidence-based practice and ongoing professional development.
• Excellent communication, interpersonal, and conflict resolution skills, with the ability to provide supervision, deliver constructive feedback, and work collaboratively within multidisciplinary teams.
• Sound understanding of the Victorian health system, experience in education or staff development, and a demonstrated ability to lead change and contribute to continuous quality improvement.
