Position Purpose

The Home Care Coordinator manages personal care services for elderly clients living at home. You will design, coordinate, and monitor individual care plans to help seniors maintain independence safely. This role involves managing government funding packages and connecting clients with support staff.

Key Responsibilities

• Client Care: Conduct assessments and create custom care plans.

• Service Coordination: Schedule support workers, nurses, and allied health professionals.

• Package Management: Administer Home Care Packages (HCP) and CHSP funding budgets.

• Family Liaison: Act as the primary contact for families and medical teams.

• Compliance: Ensure all services meet Aged Care Quality Standards.

• Roster Management: Match clients with compatible care workers efficiently.

• Review Planning: Re-assess client needs regularly to update care goals.

Skills and Qualifications

• Experience: Minimum 2 years in aged or community care coordination.

• Systems: Proficiency with client management databases and scheduling software.

• Compliance: Valid National Police Check and NDIS Worker Screening Check.

• Licence: Current Australian Driver’s Licence and reliable transport.

• Soft Skills: Exceptional problem-solving, empathy, and crisis management.
