Clinical Manager - Community Aged Care

Take the lead across clinical quality, team capability and coordinated service delivery within an established community aged care organisation. 

This senior leadership opportunity will see you provide oversight across Support at Home services, guide multidisciplinary teams and ensure clients receive safe and coordinated care that supports their independence at home.

The opportunity

Your focus will include:

• Leading, supporting and developing clinical and care teams
• Providing oversight and guidance across complex client matters
• Overseeing care plan reviews and ensuring services remain aligned with client needs
• Strengthening documentation, reporting and consistent clinical practice
• Identifying opportunities to improve processes and the client experience
• Working collaboratively with clients, families and stakeholders 

About you

We’re looking for an experienced healthcare leader who can combine strong clinical knowledge with an engaging and practical approach to leadership. 

You will ideally bring:

• Current AHPRA registration as a Registered Nurse
• Leadership experience within community aged care, home care or a relevant healthcare environment
• Knowledge of the Support at Home program 
• Strong experience across clinical assessment, referrals, case management and care planning
• Strong understanding of clinical governance, quality, safety and compliance
• Strong stakeholder management, communication and problem-solving skills
