About the Role - Tantula Rise is a Residential Aged Care Home located in Alexandra Headland on the Sunshine Coast of Queensland
As the Care Champion, you will be the frontline person, and advocate for resident's wellbeing, through checking in with all residents to ensure their comfort, care and support needs have been addressed. You will solve problems, fill gaps in planned care and support, serve as a touch point for families, keep them up to date with the care and wellbeing needs of the resident. You will help residents live a life of fulfilment and support the recruitment and training of care staff.
Responsibilities include:
Provides exceptional personalised care to residents and their families, ensures regular check-ins to address their needs, fosters open communication with all staff and maintains a comfortable environment.
Understands each resident's unique needs and preferences, provides personalised care and support, addresses any gaps in service delivery and collaborates with the care team to ensure optimal service delivery.
Communicates any changes in resident status to the Registered Nurse or Care Manager for timely intervention and delivers compassionate care aligned with the resident's personalised plan.
Welcomes new residents, facilitates introductions, ensures they are seated with peers in the dining room, follows up with families to provide updates on the resident's first day, and oversees compliance with documentation and reporting requirements.
