About The Role

We're looking for an experienced Part-Time Chronic Disease Management (CDM) Nurse to join our Bondi Medical Centre.

In this rewarding role, you'll work closely with GPs, patients, and the wider healthcare team to support the prevention and management of chronic conditions. You'll play a vital role in empowering patients to take control of their health, improving long-term outcomes, and supporting proactive, preventative healthcare.

Key Responsibilities

Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)Conduct health assessments and identify patients who may benefit from chronic disease management supportSupport and educate patients to better understand and manage their health conditionsCoach patients on sustainable health behaviour changes and lifestyle improvementsCoordinate referrals and ongoing care with multidisciplinary healthcare providersPartner with GPs to promote preventative healthcare initiatives across the practiceIncrease patient engagement in chronic disease management programsEnsure care plans are reviewed and updated in line with clinical requirementsComplete all administrative tasks associated with CDM appointments accurately and efficiently

About You

To be successful in this role, you will have:

Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN)Demonstrated experience in Chronic Disease Management within a general practice settingExperience completing health assessments (highly regarded)Strong organisational and time management skillsExcellent communication and interpersonal skills with a patient-focused approachA genuine passion for preventative healthcare and improving patient outcomesThe ability to work collaboratively with GPs and the broader healthcare team
