About the role
The Care Partner is responsible for individually planning and coordinating community aged care services in partnership with the client. This involves working closely with each client and their families to ensure they receive an individualised, responsive service under a consumer directed care model. The Care Partner will undertake assessments, develop care plans and manage an allocated client case load. The Care Partner will also be responsible for all client documentation and the maintenance of individual client budgets.
Key responsibilities
Meet with all potential HCP consumers and their families to provide information the Home Care Package service; eligibility criteria, costs and service parameters.
Ensure contracts developed for each client as per the guidelines.
Build a close working relationship with key YDHS services in particular discharge planners and service access to increase knowledge of the HCP and referral processes.
Ensure all client information obtained is accurately entered into YDHS data base and client records in a timely manner.
Complete required client documentation in accordance with policy and procedures and complete appropriate details on required data bases.
Ensure clients are informed about their right and responsibilities and provide accurate up to date information regarding privacy and use/disclosure of client information.
Provide a case management service to consumers in the program.
Apply appropriate intervention strategies including referral to Allied Health, Home Support services, Community Nursing, and external services as required to support goals identified in the care plan.
Develop and maintain a working knowledge of and links with, agencies and services within the community to facilitate utilization by program patients.
Actively participate in, or where necessary convene, case conferences/care planning meetings and external forums to ensure an integrated and effective service development response for the client.
About you
Previous experience working within the health and/or community sector.
Previous experience in undertaking assessments.
Sound knowledge of client centred services.
A thorough understanding of health and social issues that can affect the client and a demonstrated understanding of utilising resources and services available.
Knowledge of budget processes for consumer packages.
Demonstrated capacity to prioritise and manage a constant workload whilst meeting program timeframes.
Demonstrated computer literacy skills including the ability to operate in a MS Windows environment, database management and electronic information exchange, including telephone based client management.
High level communication skills at both an individual and group level with an ability to establish rapport across a diverse range of people.
Australian Qualifications Framework (AQF) Level 4 qualification as a minimum which may include a certificate IV qualification in a health and aged care discipline.
An original National Police Record Check completed within the last twelve (12) months prior to commencement.
