Care Coordinator

ACH Group has a fulltime Care Coordinator role available to provide support and enhance the quality of life of our customers based at Mile End. The role: ACH Group has a full-time opportunity for a Care Coordinator to support older people in the community and enhance their quality of life. Working closely within a multidisciplinary team you will collaborate to provide tailored service options that promote independence, choice, wellbeing, and overall quality of life for older individuals living in the community. What you’ll do: Support customers with understanding, developing and reviewing their Home Care Package Conduct assessments, develop support plans, undertake reviews, implement and facilitate clinical and care interventions Promoting ACH Groups Services and Solutions Liaise effectively with external stakeholders, including carers, acute, respite and other community services, medical and allied health professionals Provide robust budget management, ensuring that accurate budgets are maintained so the customer can be supported as required About You!
