In this role, you will build meaningful relationships with clients, provide evidence-based assessments, develop tailored treatment plans and deliver interventions that improve mobility, independence and quality of life. Key Responsibilities
Deliver high-quality, evidence-based physiotherapy services to older people in their homes and community settings, including group-based programs.
Conduct comprehensive clinical assessments and develop, implement, and review person-centred treatment plans based on individual goals and needs.
Provide interventions that support mobility, independence, participation, and overall health and wellbeing outcomes.
Maintain accurate clinical documentation, including assessments, progress notes, treatment plans, and reports within required timeframes.
Manage a clinical caseload effectively while achieving service delivery targets and key performance indicators.
Collaborate with clients, families, carers, and multidisciplinary teams to ensure coordinated and holistic care.
Educate clients and their support networks on interventions, strategies, and home programs to maximise independence.
Identify opportunities for service improvement and contribute to the development of clinical practices, procedures, and innovative models of care.
Provide support, coaching, and mentoring to less experienced physiotherapy team members.
Ensure compliance with relevant legislation, Aged Care Quality Standards, organisational policies, and workplace health and safety requirements.
About You
You are a passionate and client-focused Physiotherapist who is committed to supporting older people to live independently and achieve their goals. You will bring:
Bachelor of Physiotherapy (or equivalent) and current registration with the Physiotherapy Board of Australia (AHPRA).
Minimum 12 months post-registration clinical experience, with experience in aged care and/or community health highly desirable.
Strong knowledge of community aged care programs, client assessment, care planning, and service delivery models.
Demonstrated ability to deliver evidence-based interventions and achieve positive client outcomes.
Excellent communication, interpersonal, and stakeholder engagement skills.
Ability to work autonomously while contributing positively within a multidisciplinary team environment.
Strong organisational and time management skills, with the ability to manage competing priorities.
Experience using clinical documentation systems, CRM systems, or similar platforms.
A commitment to innovation, continuous improvement, and delivering compassionate, inclusive, and ethical care.
