Role Description We are seeking an empathetic, organised, and proactive NDIS & Aged Care Case Manager to join our team on-site in South-West Sydney (Liverpool / Fairfield West).In this role, you will be the central point of contact for participants, home care clients, and their families. You will coordinate care plans, oversee support delivery, ensure compliance under NDIS Quality and Safeguards and Aged Care Quality Standards, and build meaningful community connections across South-West Sydney.
Key Responsibilities1. Case Management & Intake:Conduct initial client assessments, onboarding, and risk assessments for NDIS participants and Aged Care Home Care Package (HCP) / Support at Home clients.Develop, implement, and review individualized Care Plans and Goal Action Plans tailored to participant needs and choices.Manage a assigned caseload, ensuring ongoing care monitoring and timely plan reviews.
2. Service Coordination & RosteringCoordinate support workers, clinical staff, and allied health professionals to deliver required services.Oversee scheduling and roster allocation to ensure client support continuity and prompt service delivery.Address and resolve day-to-day service disruptions, roster changes, or participant escalations.
3. Compliance & AdministrationEnsure accurate, timely documentation in the CRM/client management system (shift notes, care plans, incident reports).Maintain strict adherence to NDIS Quality and Safeguards, Aged Care Quality Standards, and privacy legislation.Complete monthly budget, statement, and service agreement reviews to align client plans with allocated funding.
4. Stakeholder & Community EngagementAct as the primary point of contact for families, Support Coordinators, Plan Managers, GPs, and hospital discharge planners.Represent the organisation professionally within the local Liverpool and Fairfield community.
Key Requirements & Qualifications Qualifications: Certificate IV or Diploma in Community Services, Individual Support, Case Management, or a related field (or equivalent operational experience).Experience: Minimum 1–2 years of experience in NDIS case coordination, Support Coordination, or Home Care Package (Aged Care) case management.Systems Knowledge: Familiarity with NDIS portals, myplace, or client management software (e.g., ShiftCare, or similar).Communication: Excellent verbal and written English skills; ability to communicate with diverse, multicultural backgrounds (bilingual capability in Arabic, Vietnamese, or Spanish is highly regarded in the local area).Certifications & Clearances:NDIS Worker Screening Check (or willingness to obtain)Current Working with Children Check (WWCC)National Police Check (cleared within last 6 months)Valid Australian Driver’s Licence and reliable vehicle.
